The first Survey was in1995.  It has been conducted every five years since 1995.

Yes, the online survey is conducted with Qualtrics, a secure 3rd party platform, and completed paper surveys are stored securely by the research vender, University of Wisconsin-Superior’s Center for Research and Evaluation Services.

No, you are able to skip questions – but don’t forget to hit “submit” at the end when taking the online version!

Watch your mail for a Bridge to Health Survey postcard. It will arrive in early September. If you miss that mailing, another will be mailed later in the month, followed by a paper Survey.

To thank you for your time, you will be directed upon Survey completion to a website to redeem a $5 gift card.

The Survey will take approximately 20 minutes to complete.

To ensure your answers are included in the survey results, please respond by October 31st, 2025.

Yes, the answers are de-identified and all responses are aggregated for reporting purposes.  Public use data sets do not contain addresses or other identifying information.
